knowingly transferred federal surplus food commodities in a manner not authorized by law or the rules and regulations of the Illinois Department of Public Aid in that said defendant unlawfully sold his federal surplus food commodities to Joe Brown for $500.00.

knowingly altered $500.00 worth of authorizations to participate in the federal surplus food commodities program.

knowingly transferred federal food stamps in a manner not authorized by law or the rules and regulations of the Illinois Department of Public Aid in that said defendant unlawfully sold his federal food stamps to Joe Brown for $500.00.

knowingly used $500.00 worth of altered federal food stamps.

knowingly misappropriated $500.00 in public funds made available for public aid purposes under the Illinois Public Aid Code.

knowingly possessed an altered medical card.

knowingly obtained $2,000 worth of unauthorized medical benefits in that (describe the method by which the benefits were obtained).

knowingly, by means of a false statement, obtained $500.00 in public aid under the Illinois Public Aid Code to which he was not entitled in that (describe the false statement).

knowingly solicited from Sam Smith a $500.00 kickback in return for referring Joe Brown to Sam Smith at the Standard City Health Care Center where Joe Brown could be furnished with health care benefits which would be paid for under the Illinois Public Aid Code.

knowingly solicited from Joe Brown a $500.00 kickback in return for arranging for the purchase of a building owned by Joe Brown and located at 123 Main Street, Standard City, Illinois, by the Standard City Health Care Center for which payment was to be made under the Illinois Public Aid Code.
