New Offense
In the name and by the authority of the People of the State of Illinois, , hereinafter called the complainant, on oath charges that at or about the hour of .M on or about the day of 20 in said County and State, , hereinafter called the defendant, committed the offense of:
in violation of SECTION 16G-13(a) of ACT 5 of CHAPTER 720 of the Illinois Compiled Statutes of said State, in that the said defendant, while in the course of his employment* wherein he had access to the personal information** of another person that was in possession of the State of Illinois, knowingly and with the intent of committing identity theft,*** disposed of that written**** information in a trash can***** that the public could gain access to and said defendant failed to shred that information****** so that the information was either unintelligible or destroyed.
*or official duties **whether written, recorded, or on a computer disk ***or aggravated identity theft or any violation of the Illinois Financial Crime Law ****or recorded or computerized *****or any receptable or other container ******or destroy the recording or wipe the computer disk
