COMPLAINT

In the name and by the authority of the People of the State of Illinois, , hereinafter called the complainant, on oath charges that at or about the hour of .M on or about the day of 20 in said County and State, , hereinafter called the defendant, committed the offense of:

INSURANCE FRAUD

*or attempted to obtain or caused to be obtained
**or any property
***or any insurance company or self-insured entity. For a definition of "self-insured entity" see 720 ILCS 5/46-1(d)(2)
****or by causing a false claim to be made

NOTE: Where the value of the property obtained or attempted to be obtained is $300 or less, this offense is a Class A misdemeanor. Where the value of the property obtained or attempted to be obtained is more than $300, but not more than $10,000, this offense is a Class 3 felony. Where the value of the property obtained or attempted to be obtained is more than $10,000, but not more than $100,000, this offense is a Class 2 felony. Where the value of the property obtained is more than $100,000, this offense is a Class 1 felony.
